004 Form Extension
Transitioning to the new AFF Standards required several provisional steps be taken. One of these steps was changing the time when 004 Forms were distributed.
The 004 Forms were distributed to the state committees in July 2004 rather than at the beginning of 2004. With this six month adjustment, state committees were given the extra time to complete the reinspection forms printed in 2003. The 2004 004 Forms are not due until the end of 2005. This also allows states to adjust to the new Standards and to complete their reinspections. This shift in the 004 Form schedule also influences the 2005 004 Forms, in that there will not be any reinspection forms printed this year.The next batch of reinspection forms will be printed in early 2006, at which time we will resume our yearly schedule of 004 Forms.
In summary, 2003 reinspection forms were printed in January 2003 and were not due until July 2004. In 2004, reinspection forms were printed in July 2004 and are not due until December 2005. In 2005, reinspection forms will not be printed. In 2006, reinspection forms will be printed in January 2006 and due in December 2006. As of January 2006, ATFS will resume its annual schedule for the 004 Form printing. Importantly, we will not be “doubling up” reinspections in any given year. The 004 Form cycle will adjust itself, as necessary, as time goes on.
It is intended that the extra time given for the reinspection forms currently in circulation will provide adequate opportunity for inspectors to complete inspector refresher training. If you have already completed the refresher training, please don’t hesitate to begin inspecting. Thank you for your patience and hard work in making this transition go as smoothly as possible.
